This registration session will allow you to complete contact information, sign up for participation, complete required paperwork (USA Hockey Consent to Treat / Medical History form, USA Hockey Participant Code of Conduct & MHC Player Agreement*) and submit payment through our secure site.

Please read through each page carefully to ensure that all information is provided accurately. 

* After completion of registration, you can download your Player Agreement by logging into your SportsEngine account.
