This registration session will allow you to complete contact information, sign up for participation, and submit payment. Please read through each page carefully to ensure that all information is provided accurately. To complete registration through our secure site, please have your Visa, MasterCard, Discover, or checking account information available. 

You also will need your medical insurance information. The health history / consent to treat form has been incorporated into this form.

Additional Forms: Remember that your team manager or coach will need additional forms for your player. Please print and sign them, and give them to your manager as soon as possible so that you do not delay the rostering of the team.
