This registration session will allow you to complete contact information, sign up for participation, and submit payment. Please read through each page carefully to ensure that all information is provided accurately. To complete registration through our secure site, please have your Visa, MasterCard, Discover, or checking account information available. 

You also will need your medical insurance information. The health history / consent to treat form has been incorporated into this form.

Please Note

Multi-Player Discount: To receive a discount on the second, third, and fourth players from the same family, they all must be registered together. After you register your first player, you will be given the option to register another player.  Use that option to get the discount. If you end your registration session (or register one player at a time) you will lose the opportunity for the multi-player discount.
