Fusion FC 2017 Fusion FC Recreational Tournament

Thank you for choosing to register for the 2017 Fusion FC Recreational Tournament. The following pages will take you through our application process.

If you have not previously registered online with Fusion, you will be asked to create an account below and verify your email address. An activation email will be sent to the email address you use to create the account. Once you have activated your account, please continue through the rest of the registration process to complete the forms and then check out using our secure payment platform.

When you have completed your registration, you will receive an email receipt. If you pay by credit card or checking account, you will see a charge on your account from Fusion FC. Please note that there is a transaction fee for online payment.

Coach Information (Name, Address, Contact Information)

Player Information (Name, Date of Birth, Parent Email)
