USA Hockey Registration

This registration requires a USA Hockey membership for the 2016-2017 season.

Each player must have their own unique USA Hockey confirmation number.

To obtain a USA Hockey membership, go to the USA Hockey Registration site, complete the registration, and come back to this site to complete the Flyers registration. Be sure to SAVE the USA Hockey confirmation code(s) for use with this registration.

Medical Consent Forms
