2019-20 12's/13's/14's JO Tryout Registration

Tryout Details for 12’s, 13’s, & 14’s:

Date: Sunday, November 10, 2019

Location: Hudson Prairie Elementary School

Cost: $25 plus SportsEngine Fees

Times: 14’s 10:00 – 11:30 am

                13’s  1:30 am – 1:00 pm

                12’s  1:00 – 2:30 pm

Please arrive 30 minutes prior to your scheduled tryout time to submit all required paperwork & to get sized for apparel.

Open Gym & Parent Meeting:

Date: Friday, November 1, 2019

Location: EP Rock Elementary School

Time: 6:30 – 8:00 pm

Parent Meeting: 6:30 pm EP Rock Media Center

Fee: $10 at the door

All participants must bring with them a completed & signed Medical Release Form, signed Concussion Parent & Athlete Agreement, & a copy of their USAV Trial Membership Card.  Please see USAV Membership Card Instructions below.
