Congratulations on being selected for the Colorado Thunderbirds 16U Team! 

This registration session will:

Require you to enter your 2022-2023 USA Hockey # (Which will automatically be verified through our USA Hockey live link.  If you do not have your 2022-2023 USA Hockey number, please register for it at www.usahockey.com before completing this registration)
Gather Player and Parent/Guardian information
Gather emergency and medical contact information (Used for the online Consent To Treat form)
Allow you to complete and electronically sign the following forms

- Payment Agreement

- Credit Card Payment Agreement

- Parent Code of Conduct Agreement

- Locker Room Agreement

- Parent SafeSport Code of Conduct Agreement

- Photographic and Recorded Images Consent Form

- Coach, Employee, Volunteer and Parent SafeSport Code of Conduct

- COVID Release of Liability
