This registration session will allow you to complete contact information, sign up for participation, and submit payment. Please read through each page carefully to ensure that all information is provided accurately. 2016-2017 USA Hockey Individual Membership Registration (IMR)

Go to www.usahockey.com

Click on Membership and then click register online

Click “Ice Players & Coaches”

Fill out required information

If player is under the age of 18, a parent or guardian must fill out on player’s behalf

Be sure to print your confirmation page and mail it back to the Lincoln Stars, along with your camp registration and consent-to-treat form.
